Shoot2Score Camps Registration
Please download and fill out the following form to
register for any of our Shoot2Score camps through-
out the United States and Canada. Please note, a
$75 registration fee is due along with your child's
registration. After filling out the form, pleaase print the form
and mail with your camper's deposit. The remaining balance will be due prior
to the camp's start date. In addition, please print out and mail the following waiver with
your child's registration.
